Social Worker/ Skilled Case Management

The primary purpose of this position is to triage from the point of admission to the date of discharge. As a member of the social service department, you will be responsible to follow the policy and procedure of this department in accordance with federal and state laws. As a hybrid social service person, you will assist the admission director and follow care up to and including coordination of the discharge to home or lower level of function. You must be able to multi-task and have a keen eye for detail. The case manager is the point of contact for families at a vulnerable time and you must have compassion and dedication.


  • Ability to review referrals in conjunction with, and in the absence of, the Admissions Director to screen for appropriate placement within the facility.
  • Assist in tours of the facility, can provide information to potential residents, family members and referral sources regarding the services offered
  • Gathers information and necessary Social service assessments during the Admission process that assist in the planning, coordination, and implementation of a smooth safe discharge plan within the facility, or to an alternate setting when goals have been met.
  • Coordinate care needs and clinical information as appropriate to various departments and IDT members to ensure a seamless admission from the community or hospital into the facility
  • Maintains positive communication with family members and referring facilities
  • Works independently to promote the mission of Lifequest Nursing Center
  • Document in accordance with Federal and State laws
  • Work as a member of a multi-disciplinary team
  • Attend Care Conference meetings
  • Build relationships with referral partners


  • Experience in a Skilled Nursing Facility setting (admissions, business office, social services, Nursing etc.)
  • Clinical experience a plus
  • Knowledge of medical terminology
  • Excellent communication, time management and organizational skills
  • Must communicate effectively to all inquiries in a respectful, appropriate and timely manner.
  • Working knowledge of social service practice in long term care

  • Long term care setting experience (2-3 years) working with admissions and discharges.
